Endolift journal · Compare · 4 min readEndolift or filler for jowls: which problem are you treating?

Endolift and filler treat different causes of jowls. Endolift tightens mild to moderate loose skin and reduces small fat pockets along the jaw. Filler restores lost volume and shape, often in the cheeks or along the jawline. If your jowls come from loose skin and fat, Endolift fits; if from lost volume, filler may. Results vary.

Why jowls form

Jowls are the soft fullness that appears beside the chin and blurs the line of the jaw. They rarely have a single cause. With age, skin loses collagen and becomes looser. Fat along the lower face can settle downwards. At the same time, the cheeks and the area around the jaw can lose volume, so the support above the jowl weakens and the jowl looks heavier.

So when someone asks whether Endolift or filler is better for jowls, the honest answer is that they treat different parts of the problem. Endolift tightens and reduces small fat pockets. Filler replaces volume and adds shape. Choosing well means working out which change has happened most in your face.

A few simple observations help. If you gently lift the skin in front of the ear and the jowl largely disappears, loose skin is a major factor. If you pinch the jowl and feel a soft pad, fat is playing a part. If your cheeks look flatter than in photos from 10 years ago, volume loss is likely to be involved. Most people see a little of each, and a clinician should explain the balance in your face before suggesting anything.

How Endolift treats jowls

Genuine Endolift uses the LASEmaR 1500 laser by Eufoton at 1470 nm, with original single use fibres. After local anaesthetic, a fibre between 200 and 600 microns wide is placed beneath the skin through a tiny entry point, with no incisions and no stitches.

The laser contracts the fibrous septa under the skin, which gives some tightening immediately. It can liquefy small fat deposits in the jowl, and it triggers collagen remodelling, as weaker Type III collagen is replaced by stronger Type I. The jawline therefore keeps improving for around 6 months. Treatment takes about 60 to 90 minutes, and most people need 1 session. More detail is on the jowls and jawline page, and a week by week account is in Endolift for jowls: what happens week by week.

Endolift does not restore lost volume. If the main problem is hollowing, tightening alone can leave the face looking a little flat.

How filler treats jowls, in general terms

Dermal filler is injected to replace or add volume. For jowls, it is sometimes placed in the cheeks to restore support from above, or along the jawline and chin to create a straighter line and disguise the jowl. Done well, it can make a clear difference to shape.

Filler does not tighten loose skin or remove fat. If too much is used to hide a heavy jowl, the lower face can look wider or heavier rather than younger. Filler also wears off over time and is topped up. Ask any clinic offering filler what product it uses, how much it plans to use, who injects it and what the risks are.

Combining them, and the question of timing

Many faces benefit from both: tightening along the jaw and a modest amount of volume where it has been lost. When they are combined, the order and timing matter. Endolift works by heating tissue beneath the skin, so recent filler or threads in the same area need discussion first. Tell your clinician about any filler you have had, where it was placed and when.

Where jowls are heavy and the skin hangs, neither treatment is a good answer. A surgical facelift usually gives a clearer, longer lasting result, and in London costs £7,000 or more. The Endolift vs facelift page explains where the line usually falls.

Recovery, risks and cost

After Endolift along the jaw, expect swelling, bruising, redness and tenderness. The skin can feel numb, and treated fat can feel firm or lumpy. Most of this settles within days, although numbness and firmness can take 1 to 2 weeks or longer. Most people return to work the next day or within 48 hours.

Rarer risks include burns, skin irregularity, infection, scarring at the entry point and temporary nerve weakness. Along the jaw, irritation of the marginal mandibular nerve can affect the smile for a period. These are uncommon but should be explained before you agree to treatment. See the side effects and safety page for detail.

At TK Medispa in Kensington, the jowls alone are priced as a single face area at £1,500. The lower face (lower cheeks, jowls, chin and neck) costs £2,000. Prices include the photo assessment, video consultation, treatment, local anaesthetic, aftercare and review. All areas are listed on the cost page. Results can last up to 2 years, depending on age, skin, weight stability, sun exposure and smoking. Results vary.

Dr Seth's view "I look at the face from the front and from the side, and I ask whether the jowl is there because something has dropped or because something has been lost. If the skin and fat have loosened, Endolift makes sense. If the cheek has flattened, adding a little volume may help more. Often it is both, and I would rather explain that than sell a single answer."

Questions people also ask

Can filler make jowls look worse?

It can if it is used to hide a heavy jowl rather than to restore volume that has been lost. Too much filler along the jaw can make the lower face look wider or heavier. Used carefully and for the right reason, filler can improve shape and support.

I have filler in my jaw. Can I still have Endolift?

Possibly, but it needs discussion. Recent filler or threads in the treatment area are a reason to pause, because Endolift heats tissue under the skin. Tell Dr Seth what product was used, where and when, and he will advise on timing.

Which lasts longer for jowls, Endolift or filler?

They are not directly comparable, because they do different jobs. Endolift results can last up to 2 years, depending on age, skin and lifestyle. Filler lasts for a period that varies with the product and the area, and is then topped up.

Are jowls in your 40s treated differently from jowls in your 60s?

Often yes. Earlier jowls tend to involve mild laxity and small fat pockets, which suit tightening. Later jowls may involve more volume loss and heavier skin, where filler, surgery or a combination may be needed. Age is a guide, though, and skin quality matters more.
