Endolift journal · Areas · 4 min readOzempic face: loose skin after weight loss injections, and what helps

Ozempic face describes the loose, tired look some people notice after rapid weight loss on injections such as semaglutide. It combines lost facial volume with skin that has not shrunk back. Endolift can tighten mild to moderate laxity in the jowls, jawline and neck, but it does not restore volume, and heavier loose skin may need surgery.

What people mean by Ozempic face

Ozempic face is not a medical diagnosis. It is a popular term for changes some people notice after losing weight quickly with injections such as semaglutide. Friends may say they look tired or older, even though they feel healthier. The same changes can happen after any substantial weight loss, whether from injections, diet, exercise or surgery.

The term covers 2 different problems that often appear together. The first is lost volume: the fat pads in the cheeks and around the eyes shrink, so the face looks flatter or hollow. The second is laxity: the skin, stretched over a fuller face for years, does not shrink back fully, so it sits lower around the jaw and neck. Knowing which of these is your main concern is the most important step in choosing a treatment.

Why the skin does not always shrink back

Skin contains collagen and elastin, which give it strength and spring. When weight is lost slowly, younger skin with good collagen often adapts. When weight is lost quickly, or when skin is older, sun damaged or has been stretched for a long time, it has less ability to recoil. The result is skin that drapes rather than fits.

In the face, this tends to show along the jawline as jowls, under the chin as a soft or crepey area, and in the neck as loose bands or folds. In the body, the stomach, upper arms and inner thighs are common areas. Our page on loose skin after weight loss covers the body in more detail.

What Endolift can do

Endolift uses a 1470 nm laser delivered through a very fine fibre placed under the skin through a tiny entry point, under local anaesthetic. The energy contracts the fibrous septa that connect skin to deeper tissue, which gives some immediate tightening. It also triggers collagen remodelling, where weaker Type III collagen is replaced by stronger Type I collagen, so the skin continues to firm for around 6 months.

For someone after weight loss, that can help with:

  • Mild to moderate looseness along the jawline and jowls.
  • Soft or slightly loose skin under the chin.
  • Small remaining fat pockets, which the laser can liquefy.
  • Early laxity in the neck.

It is usually a single session of about 60 to 90 minutes, with no incisions or stitches. Most people return to work the next day or within 48 hours, although swelling and bruising are common at first.

What Endolift cannot do

This part matters, because many people with Ozempic face are disappointed by treatments that were never designed for their main problem.

ConcernCan Endolift help? What usually suits better
Mild to moderate jowls and neck laxityOften yesEndolift is a reasonable option
Hollow cheeks or templesNoVolume restoring treatments, discussed with a doctor
Heavy, hanging skin of the neck or faceLimitedSurgical facelift or neck lift
Large amounts of remaining fatLimitedFurther weight management or liposuction

Endolift does not add volume. If your face mainly looks hollow, tightening the skin alone may not give you the change you want, and in some cases could make the face look leaner. If you have lost a very large amount of weight and the skin hangs in folds, surgery is usually the more effective route. The comparison of Endolift and a surgical facelift explains where the line usually falls.

When to consider treatment

Timing matters. Endolift results last longest when weight is stable. If you are still losing weight, the skin will keep changing, and the fat the laser treats may still be shrinking anyway. Most doctors would suggest waiting until you have reached a steady weight and held it for a while before treating loose skin.

It is also sensible to tell the doctor which medicine you are taking, the dose and how long you plan to continue. Weight regained after treatment can stretch the skin again, and further weight loss can reveal more laxity. Neither is a reason to avoid treatment, but both affect how long results are likely to last, which is up to 2 years.

Dr Seth's view "Many patients who come to me after weight loss injections are delighted with their health but unhappy with their jawline and neck. The first thing I do is separate volume loss from loose skin, because they need different treatments. If the main issue is mild to moderate laxity around the jaw and neck, Endolift can help. If it is hollowing, or skin that really hangs, I will tell them, because a laser under the skin will not fix that."

Risks and recovery to expect

Expected effects after Endolift include swelling, bruising, redness, tenderness, temporary numbness and temporary firmness or small lumps where fat was treated. Most settle within days, although numbness and firmness can take 1 to 2 weeks or longer. A compression strap is usually worn on the neck for 3 to 4 days. Rarer risks include burns, skin irregularity, infection, scarring at entry points and temporary nerve weakness, for example affecting the smile. The side effects and safety page explains each of these plainly. Results vary between people.

Questions people also ask

Should I stop my weight loss injections before Endolift?

That is a decision for you and the doctor who prescribes them, not your aesthetic doctor. What matters for Endolift is that your weight is reasonably stable before treatment, so the skin is not still changing. Tell the treating doctor what you take and your plans.

Can Endolift fix hollow cheeks after weight loss?

No. Endolift tightens skin and can reduce small fat pockets, but it does not add volume. Hollow cheeks or temples need a different approach, so it is important to have the cause of your concern assessed before choosing a treatment.

Will loose skin come back if I lose more weight?

Further weight loss can reveal more laxity, and regaining weight can stretch the skin again. Endolift results last longest with stable weight. They can last up to 2 years, depending on age, skin quality, sun exposure and smoking.

Will Ozempic face go away on its own?

Some of the change may improve slightly over time as skin adapts, especially in younger people. However, lost facial fat does not usually return on its own unless weight is regained, and stretched skin often stays somewhat loose without treatment.
